As a new patient here’s what to expect.

Before I can start treating you I need to gather some background information from you. This will ensure that you treatment is truly tailored to your individual need.

To gather this information we have some patient forms which can be downloaded below and completed in the comfort of your own home. Please remember to bring them with you on your first appointment.

  • All forms are PDF files. You will need AdobeReader® to view them.
  • If you do not already have AdobeReader® installed on your computer, Click Here to download.
  • Download the necessary form(s), print them out and fill in the required information.
  • You may scan and email your completed form(s) or bring it with you on your first appointment.

Your First Visit:

If this is your first time visiting us we will need to spend a bit more time with you. The first appointment is between 30-45 minutes long. We will be asking you about your current problem and overall health, as well as take a detailed medical history. Because we view you as a unique human being we will ask you what your health goals are and what you hope to get out of your chiropractic adjustment sessions. We need to make sure that chiropractic is the right treatment option for you. We also need to make sure we can come up with a treatment plan that meets your expectations and goals.

For the physical examination we’ll be asking you to move quite a bit. So we ask that you wear clothes that are nice and loose so that you can wiggle about and easily bend down to touch your toes. Ladies, please wear a sports bra so we can easily examine your spine.

After completing the examination sometimes we need more information.  So we might send you for and x-ray or a scan, or need to chat to your GP. This is only if indicated.
At the end of your first appointment be will book you in for your Second Visit/ Report of Findings.

(In some instances you may be able to have your first treatment after your examination.  This however is at the discretion of the chiropractor.)

Second visit -The Report of Findings

If your Report was not done at the first visit it will take place at the second appointment.  Each and everyone of us is unique.  At this appointment we will review your findings from your first appointment and come up with a treatment plan that matches not only your problem but also your health goals.  Our goals at Celtic Family Chiropractic are to help you reach your highest potentially physically, mentally and emotionally.  The care plan for someone looking for a quick fix will be very different to someone who’s goals are to optimise performance and prevent injuries.

Once we have agreed on the best treatment plan for you then you are ready for your first adjustment.  Let the healing begin!
